Your First Visit

Your initial appointment will consist of a thorough examination with an explainition of your diagnosis and treatment options. Dr. Chunn believes that the best service that he can provide to his patients begins with a thorough examination of the head and neck region. This includes a cancer screening, gum exam, and an evaluation of the teeth as well. He will address all of your concerns and make his recommendations so that your objectives are accomplished. During the examination process, appropriate x-rays are taken and a treatment plan is formulated for you before any treatment begins. Your treatment plan will show how long the treatment will take and how much it will cost before any treatment begins. We can estimate insurance coverage for you as well. Our office is very thorough about making sure you understand all aspects of the care that we provide.   Occasionally, treatment can be done the same day as the consultation. However, a complex medical history or treatment plan will require a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

  • Any recent X-rays if applicable (Complete sets of x-rays or Panoramic film when less than 3 years old)
  • A list of medications you are presently taking
  • If you have medical or dental insurance, bring the necessary information. This will save time and allow us to help you process any claims.
  • All of these items may be taken care of online prior to your visit if more convenience for you.

IMPORTANT: All patients under 18 must be accompanied by a parent or guardian during the consultation visit. Please alert the office if you have a medical condition that may be of concern (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.); a list is helpful.

X-Rays

If your previous dentist or physician has taken x-rays within the past 3 years, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.
